Rates and Insurance

FGC Counseling Services utilizes a fee-for-service model of payment which means that they are not contracted with health insurance providers.  If you have out-of-network mental health coverage, your insurance company may cover a significant portion of the session fee.  Please contact your insurance company directly to inquire about “out-of-network mental health coverage” to determine your out of pocket cost per session.

Fees for Services

Fees for services are dependent on the service and the provider for which you work with. Please contact the office to inquire about specific fees for initial consultation, regular therapy session, workshops and consulting services.​

Payment

FGC accepts cash, check and all major credit cards as forms of payment.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you will be responsible for the full rate of the session.

Insurance

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Many health plans will reimburse for treatment provided by licensed providers, even if they are not considered “in-network” or “preferred providers.” FGC can provide you with a monthly statement to submit to your health insurance provider for reimbursement purposes.

Please contact your insurance provider to verify how your plan compensates you for psychotherapy services.  We recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan have out-of-network benefits?

  • If yes, does this policy cover out-of-network psychologists or Mental Health Counselors?

• How much will my policy pay for the following coded sessions (used most commonly in this practice)? 90791, 90834, 90846, 90847 and 90853.

• Do I have a deductible? If so, what is it and have I met it yet?
